What the Examiner Expects
Definitive treatment of an anal fistula by laying open the fistula tract (fistulotomy) or using sphincter-preserving techniques for complex fistulas. The examiner expects you to know Goodsall's rule for predicting internal opening location, classify fistulas using the Parks classification (intersphincteric, transsphincteric, suprasphincteric, extrasphincteric), and determine which fistulas are safe for fistulotomy vs those requiring sphincter-preserving techniques. A simple fistulotomy is appropriate for intersphincteric and low transsphincteric fistulas involving less than 30% of the external sphincter. For high transsphincteric fistulas, options include cutting or draining seton, LIFT procedure (ligation of intersphincteric fistula tract), or endorectal advancement flap.

This presentation should raise concern for Crohn's disease. Obtain colonoscopy, MRI pelvis. Crohn's fistulas are managed primarily with medical therapy (biologics — anti-TNF agents like infliximab) with seton drainage. Aggressive fistulotomy in Crohn's leads to non-healing wounds.
